Family Dental Associates Membership Club
We help uninsured patients by providing affordable payment solutions suitable for their needs. When you join our membership club, you can receive your preventive dental care in Miami, Oklahoma, for low monthly payments, as well as other needed treatments at a discounted rate. We currently provide three payment options under our Membership Club, learn more below.
Membership Club Options
Patients 12 and below will receive the following treatments at $33 per month with a lifetime activation fee of $99.
- Two professional dental cleanings per year
- Two dental exams per year
- Any needed X-rays
- Fluoride treatment
- One emergency exam per year
- 15% off all other treatments (some exclusions may apply)
Patients aged 13 and older will receive the following under our adult program for $43 a month with a lifetime activation fee of $99.
- Two professional dental cleanings per year
- Two dental exams per year
- Any needed X-rays
- One emergency exam per year
- 15% off all other treatments (some exclusions may apply)
*Note: membership applies to patients with regular cleanings and the absence of periodontal disease.
Patients 13 years old or older will receive the following treatments under the Perio Membership for $63 per month with a lifetime activation fee of $99.
- Periodontal maintenance cleanings (3-4 per year)
- Two dental exams per year
- Any needed X-rays
- One emergency exam per year
- 15% off all other treatments (some exclusions may apply)
*Note: membership applies to patients enrolled in active periodontal maintenance due to prior treatment for periodontal gum disease.
