Dr. Mark Folks and his wife, Kim, founded Family Dental Associates in 2002 with just two operatories and three employees. Today, the practice has expanded to include 14 operatories and a dedicated team of 30 employees, serving Northeast Oklahoma and the Four State Area. Dr. Folks is passionate about giving back to his community and transforming lives through dental health.

Raised in Midwest City, Oklahoma, Dr. Folks attended Southwestern Oklahoma State University in Weatherford, where he played football for four years and earned a Bachelor of Science degree in Biology. It was at Southwestern that he met his wife, Kim, who was pursuing a pharmaceutical degree. Dr. Folks then attended the University of Oklahoma College of Dentistry, graduating in the top 10% of his class. He further honed his skills through a one-year Advanced Education in General Dentistry residency and has continued to expand his expertise through organizations like the American Academy of Cosmetic Dentistry, the Academy of General Practice Orthodontics, and the Academy of Implant Dentistry. He has also received advanced training in oral conscious sedation.

Over the years, Dr. Folks has served on numerous boards and committees, including the Miami Schools Board of Education, the Miami County Facilitation Authority, the Rotary Club, and the Oklahoma Dental Association Board of Directors. He is also a member of the University of Oklahoma College of Dentistry Admissions Committee. Active in his church, Dr. Folks teaches Sunday school and has been a part of the church's leadership committee for many years.

Dr. Folks has a true love for education and mentoring, which is evident in his role as an instructor for the dental assistant course at Northeast Tech for the past two years. His passion for teaching and guiding the next generation of dental professionals is a testament to his commitment to the field.

Both Dr. Folks and Kim have a strong passion for giving back, extending their service beyond their employees, patients, and local community. Their dedication was recognized when they received the Boys and Girls Club of Ottawa County Community Builder of the Year Award in 2022, and Family Dental Associates was named Business of the Year by the Community Crisis Center in 2024.

Dr. Mark Folks and his wife, Kim, are proud parents of three sons. Christian, Caden, and Carson all graduated as valedictorians from their respective high school classes. Christian pursued a BSBA in Finance at the University of Arkansas and obtained his law degree from the University of Denver Sturm College of Law, now excelling as an attorney in the oil and gas industry. Caden, a graduate of the University of Arkansas with a degree in Environmental Science and a minor in Agricultural Business, has taken to the skies as a pilot. Carson is currently in his senior year at the University of Arkansas, studying Civil Engineering.

Dr. Matthew A. Keim has been a dedicated member of Family Dental Associates for 11 years, following a decade of owning a practice in Miami, Oklahoma. With a passion for service, Dr. Keim has been actively involved in his community, including serving on the Neosho Christian School Board for 12 years and participating as a member of Rotary. His commitment to helping others extends beyond dentistry, as he finds fulfillment in serving on mission fields and participating in community events like serving Thanksgiving dinner to those less fortunate alongside fellow believers at his church.

Dr. Keim earned his bachelor’s degree in high school science education before pursuing his Doctor of Dental Surgery (DDS) degree at the University of Oklahoma College of Dentistry, graduating in 2003. He is committed to lifelong learning, regularly advancing his dental education by exceeding state requirements and staying informed about the latest advances in dentistry. Dr. Keim has received advanced training in implant dentistry and oral conscious sedation and is a proud member of the American Dental Association.

In his practice, Dr. Keim's goal is to help patients achieve optimal overall health by providing the most advanced and effective treatments. He is dedicated to not only ensuring excellent oral function but also enhancing his patients' confidence with beautiful smiles.

Outside of the office, Dr. Keim enjoys fishing and spending time in the outdoors. He is a proud father of three sons: Blake, who manages restaurants in Dallas; Jackson, who is a pilot; and Chance, who is currently attending classes at Crowder Community College.

Dr. Mallory Sartor is a dedicated dentist with a passion for serving others, particularly those in underserved communities. A native of Paris, TX, Dr. Mallory graduated from the University of Oklahoma College of Dentistry with Outstanding Distinction, where she was the only graduate to achieve a perfect 4.0 GPA. Her commitment to excellence is matched by her heart for service, which has led her on mission trips to Haiti, Mexico, and Africa to provide dental care to those in need.

Before pursuing dentistry, Dr. Mallory earned a degree in Nutritional Sciences from Oklahoma State University, graduating as a Top 10 Senior in the College of Human Sciences. She has always been drawn to the intersection of health, wellness, and community, and believes deeply in the power of dentistry to make a lasting impact on individuals and families.

Dr. Mallory is proud to call Miami, OK, her home, feeling called to serve the local community and beyond. She is committed to providing high-quality care in a compassionate and personalized manner, building lifelong relationships with her patients that span generations. Her love for dentistry is fueled by her genuine passion for connecting with people and making a positive difference in their lives.

Outside of the office, Dr. Mallory enjoys spending time at the lake, playing couples golf with her husband, and cheering on her Oklahoma State Cowboys. She is excited to continue her journey of service and care in the Miami area, where she strives to make every patient feel like family.

Kim Folks is an extraordinary Human Resources and Benefits Coordinator, celebrated for her unwavering compassion, kindness, and dedication to nurturing meaningful relationships. Her ability to recognize and celebrate the unique gifts and talents of others makes her an indispensable member of our team. Kim's joy in collaborating with her husband to create a positive impact on their employees and community is truly inspiring.

A proud native of Afton, Oklahoma, Kim began her academic journey at NEO A&M College, where she earned her associate's degree. She then pursued a Bachelor of Science degree in Pharmacy at Southwestern Oklahoma State University. As a pharmacist at Osborn Drugs in Miami, Kim is deeply committed to educating and caring for her patients with the utmost dedication.

Kim and Dr. Folks recently marked a remarkable milestone, celebrating 30 years of marriage. Together, they have raised three exceptional sons: Christian, Caden, and Carson, all of whom graduated as valedictorians from Miami High School. Kim's commitment to her community is unparalleled. She has served on numerous boards and is passionately devoted to volunteering. Her efforts were recognized with the prestigious Mark Rogers Volunteer of the Year award in 2022. Kim and Dr. Folks were also honored with the Boys and Girls Club of Ottawa County Community Builder of the Year award in 2022. Her instrumental role in Family Dental Associates being named Business of the Year by the Community Crisis Center in 2024 is a testament to her dedication. Additionally, Kim is an integral part of the FDA Mission team, traveling to Costa Rica to provide dental care to those in need.

Kim's passion for people and her boundless love defines her servant's heart, making her a beacon of inspiration and positivity within our organization and beyond.

Paige is a dedicated wife, loving mom, and skilled aesthetic nurse with a passion for helping others feel confident and beautiful. Balancing family life with a career in the beauty and wellness industry, she combines her expertise in skincare and aesthetics with a genuine care for her clients’ well-being. Whether at home or in the clinic, Paige strives to create a positive impact on everyone she meets.

Julie has been with Family Dental Associates since 2014. Prior to joining FDA, Julie worked for Dr. Keim for 11 years at his practice downtown. She graduated from Commerce High School and attended NEO and OSU on a softball scholarship. Julie graduated from the Missouri Southern State University Dental Hygiene program in 2003. She has recently been promoted to Hygiene Team Lead and works hard to educate, inspire, and encourage her team to give our patients the best possible care. Julie loves leading a team that works together toward the same goals: educating patients about the health of their mouths and how that affects their overall physical health all the while creating beautiful smiles.

Julie and her husband, Scott, are celebrating 25 years of marriage this June. They enjoy spending time outdoors with their kids and grandbaby.

Andrea Mehrens joined Family Dental Associates Miami OK as a Dental Hygienist in January 2022. She attended East Newton High School before graduating from MSSU's Dental Hygiene Department in 2010. Andrea works hard to make sure that all our patients have a great experience and gain knowledge at each of their appointments. She and her husband, Mark, have one son named Joby. They live in Seneca, Missouri and love spending their free time outdoors.

Erin has been with Family Dental Associates since the fall of 2015. She graduated from Miami High School in 2000 and earned her Bachelors in Biology from Oklahoma State University in 2003. Fun fact, anything orange and black in the office makes her smile, GO POKES! Erin graduated from Missouri Southern with a degree in Dental Hygiene in 2011. She loves being able to work within the small town that she grew up in. Erin works alongside our incredible Hygiene Team to ensure that our patients receive the quality care and the knowledge they need to maintain healthy teeth and gums. Erins favorite thing about her job is helping our patients create beautiful smiles that they are proud of.

Erin and her husband, Jason, have been married since 2013. They were blessed with twins, Aiden and Brynlee, in the summer of 2018. Outside of the office Erin enjoys spending time with her family, dragging her husband shopping and seeing new growth within our town!

Faith joined the Family Dental hygiene team in the Spring of 2021. She is from Miami and graduated from Miami High School in 2013. She studied at MSSU, graduating with a bachelors in health science and an associate degree in dental hygiene. Faith and her husband, Jason, have been married for nine years. Together they have two beautiful girls, Eden and Ellie. They are members at Quapaw First Baptist Church and love helping in the community whenever they can.

Mika has been with our team as a dental hygienist since 2015 but her history with Family Dental started when she was in high school. Mika came to the practice with no dental knowledge and quickly became a dental assistant. She graduated from Miami High School in 2005, and attended Missouri Southern State University for dental hygiene, graduating in 2010. In 2015 we welcomed Mika onto the hygiene team.

Mika is one of the most compassionate people you will ever meet. She loves her profession and the relationships she has with every patient. Mika has a giving heart, always willing to help others in any way. She is proud to say that she works with some great ladies and wonderful doctors, she says they are more than coworkers they are family.

Mika and her husband, Brandon, were married in 2008 and have two children, Hudson and Karson. She enjoys spending time with her family. Between her active children and their home art business we don't know how she does it all. She is active in her church and is the secretary of the Miami Enrichment Foundation. Come meet Mika, she will take wonderful care of you and treat you like Family.

Taylor joined Family Dental Associates Miami OK, as a Dental Hygienist, in January 2022. She is originally from Wichita, Kansas. She graduated from Wichita State University's Dental Hygiene Program in 2017. She spent the next four years at a Dental Practice in Fort Worth, Texas. She strives to ensure that our patients receive the quality of care and knowledge of the needs to maintain healthy gums and teeth. Taylor is excited to be working with our fabulous team and doctors to make beautiful smiles. She enjoys spending time with her Husband Lucas, and 3 kids, Saige, Clyde and Teagan. They love to travel, fish, and ranch.

Felicia started at Family Dental Associates in March 2024, but her dental experience doesn’t start there. She graduated from Grove High School in 2012 and immediately went to Northeastern State University working towards a bachelor’s degree. She was accepted into hygiene school early, so she transferred and graduated from Missouri Southern State University in 2018 with a bachelor’s in general studies and an Associates in Dental Hygiene. She worked as a dental hygienist in Grove for 6 years before starting here! Felicia enjoys getting to know her patients and educating them about their oral hygiene.

Felicia married Clayton in 2020, and they have two children, Timothy and Wyatt. She enjoys spending time with her family and going to church for Sunday service and Wednesday night bible study!

With over 15 years of dedicated experience, Kinzie is a cornerstone of our dental team, known for her exceptional skills and compassionate patient care. As the Lead Dental Assistant, she is passionate about mentoring her team and fostering an environment that reflects our core values: Fun, Teamwork, Create Value, Positivity, Greater Good, Results Oriented, and Ethical practices.

Kinzie believes in the power of friendships and building strong relationships, both with her colleagues and patients. Her positive energy and commitment to excellence make her an invaluable asset to our practice.

Outside of work, Kinzie is a devoted wife to Cory, with whom she has shared 11 wonderful years of marriage. Together, they have two energetic boys, Kameron and Kynden, who keep them busy with their involvement in every sport imaginable. Whether on a ballfield or a ballcourt, Kinzie cherishes every moment spent with her family.

Erin Krause joined our practice as a chairside dental assistant in April 2021. She graduated from Vinita High School in 2009 and later attended Green Country School of Dental Assisting. Erin wakes up every day excited to work alongside our excellent Doctors and a team full of talented ladies, who have all become more like family! Erin and her husband, Brandon, share four beautiful children. Her big, blended family spends most of their time on a ballfield or court, as the children play ALL the sports. They do enjoy family movie nights when time allows.

Laney joined Family Dental Associates as a chairside dental assistant in June 2021. Making patients feel comfortable during appointments is her number one priority. Laney loves seeing the results of each patient's case. Watching our patients become so confident with their smiles and knowing that she had the pleasure of playing a role in someone's confidence is the most rewarding feeling to her. Laney is married to her husband Gabe, and is a mother to one sweet boy, Greyson. She has two fur babies, Dixie and Cash, which she adores!

Dionne joined Family Dental Associates as a dental assistant in February 2023. She is proud to be a part of a hardworking and passionate team. Dionne is excited to learn, to grow, and to serve her community one patient at a time.

In Dionnes free time she enjoys taking care of her chickens, being outdoors and thrifting vintage clothing!

Makayla joined the FDA team as a chairside dental assistant in June 2024. She graduated from Miami High School in 2020 and went on to cheer in college at both NEO and MSSU. Makayla then decided to pursue her dream of working in the dental field by attending the dental assistant course through Northeast Tech, Dr. Folks was her instructor. She is very excited to continue both learning and growing in the amazing environment at FDA. In Makalya’s spare time she enjoys being around her family and her dog, Finley.

Michelle has been an essential part of our team since 2012, serving in multiple key roles, including Front Office Team Lead, Treatment Coordinator, and an active participant in onboarding all new team members. She is passionate about coaching and motivating her colleagues, helping them to achieve their personal and professional goals, while also showing new team members how to thrive in our office culture. Michelle's greatest joy comes from inspiring patients to move forward with life-changing treatments. As she puts it, “We’re not just providing exceptional care; we are changing lives by building meaningful relationships.”

Michelle has been happily married to her husband Zack since May of 2009, and together they are raising two active boys who are involved in football, basketball, powerlifting, track, baseball, and a love for the outdoors. When Michelle isn’t at the office, you’ll likely find her spending quality time with her boys, whether it's in a tree stand hunting or fishing at a nearby pond. Alongside Zack, Michelle enjoys hosting cookouts and serving up a good meal for friends and family. Above all, Michelle is deeply devoted to her family and friends, cherishing every moment with them.

Mallorie joined Family Dental Associates as an insurance coordinator in August 2021. She came to us with years of dental experience and insurance knowledge. Mallorie and her husband, Dylan, share an adorable boy named Merritt, who turns 3 in March. They also have two fur babies, Gyntri and Sage. Outside of the office, Mallorie stays busy running a jewelry business, Rafter W Design Co., where she creates unique pieces, mainly of Navajo pearls and turquoise.

Mika joined our team in October of 2016 and has several years of experience in working with dental insurance. She graduated from Miami High in 2002 and from NEO in 2004 before studying at Missouri State University. She loves our patients and works hard in handling insurance so that our patients do not have to deal with it themselves.

Mika and her husband, Robert, have two daughters named Carolina and Livingston. Mika and her family are beekeepers and enjoy a little farm, garden, cows, sheep, goats, cats, and dogs outside the office. She also dabbles in winemaking and brewing and enjoys ringing the bell for the Salvation Army and Hearts & Hammers. Mika is an active member of the FUMC of Miami.

Carmen Dintelmann is a senior at Grand Canyon University, where she is pursuing a degree in Marketing and Advertising with a minor in Entrepreneurship, all while studying online. She spent three years on campus in downtown Phoenix, gaining valuable insights and experiences that have shaped her approach to both work and life. Originally from Vinita, Carmen is excited to be back home, spending time with her family and two nephews.

An adventurous spirit, Carmen loves exploring new places and pushing her limits—she's gone skydiving, traveled across the country, and ventured around the world. This passion for exploration is mirrored in her dedication to making every patient feel heard, valued, and cared for. Carmen is committed to creating a warm and welcoming environment where each person feels genuinely loved and listened to during their visit.

Alayna joined Family Dental Associates in January 2024, bringing a passion for providing high quality dental care to every patient. With a commitment to helping patients maintain their oral health and achieve beautiful smiles, Alayna is dedicated to creating a welcoming and comfortable environment.

Outside of the office, she has been happily married since 2010 and is the proud parent of two children. Living in Afton, OK, Alayna enjoys spending time with family, as well as exploring the local community.

Cecilia has been with Family Dental Associates Miami OK since 2017. She acts as a treatment coordinator, and she is an incredible asset to our front office, as she is bilingual. Cecilia creates a warm, welcoming atmosphere for all our patients. She enjoys making each patient feel valued.

Cecilia and her husband, Andy, have been married since 2006. They have two boys, Louie and Bishop, who are very active in sports. She enjoys spending time with her family outdoors. Cecilia is highly artistic and enjoys crafting.

Kelsey joined Family Dental Associates in March 2022, bringing over a decade of customer service experience to the team. A proud Fairland native, Kelsey graduated from Fairland High School in 2015. She went on to earn an associate’s degree from NEO A&M in 2017 and completed her bachelor’s degree in business administration at Rogers State University in 2020. Kelsey is passionate about serving her community, both through her work and her involvement as a member of Women in Business with the Miami Regional Chamber of Commerce.

Outside of work, she and her husband, Dillon, enjoy cheering on the Kansas City Chiefs—whether at home or at Arrowhead Stadium. In addition to being a die-hard football fan, Kelsey is a dedicated aunt, always on the sidelines at her nephews’ sporting events. Family is one of Kelsey’s greatest joys and she loves spending time with her loved ones.

Kelsey takes pride in building meaningful relationships and looks forward to continuing to make a positive impact in the lives of her patients and her community.

Angie joined the FDA team in January 2024. She has over 25 years of customer service experience, a field that she loves because it allows her to build relationships and connect with people. Angie loves being part of the FDA because of our amazing and supportive team! She is excited to serve our patients and establish long-lasting relationships with them.

Angie has been married to her husband, Mike, for 29 years. They share a daughter, Ashley, who works as a pediatric dental hygienist in Joplin. In September they celebrated their granddaughter, Hazely’s 1st Birthday! Angie is grateful to spend her days off with Hazely, as being a GiGi brings her great joy. On Wednesday nights she teaches a girl’s class at First Assembly of God in Afton. She loves teaching and watching the girls learn about Jesus! In Angie’s spare time she loves being a GiGi, reading, crafting, gardening, spending time outside, watching the sunset, making clay earrings and photography!

Brianne has been with Family Dental Associates since 2007, when she moved from Kansas City, MO after marrying her husband, Tyson. Her family is very active in their church, Lake Center Baptist on Monkey Island, where Tyson serves as their Student Pastor. Brianne is our Hygiene Coordinator and Culture Creator who has a passion for creating a positive and collaborative workplace culture. With a focus on team-building, patient care and continuous improvement, she works to inspire a supportive environment where both staff and patients thrive. Her dedication to fostering a culture of excellence drives her commitment to high standards and compassionate care. She believes in the power of small acts to create big, lasting impacts.

Dana joined the FDA team in May 2015 as a Scheduling/Treatment Coordinator. Over the years, her role has evolved into that of Patient Experience Coordinator and Marketing Director. Dana is deeply passionate about people and is dedicated to ensuring that every patient feels seen, valued, and appreciated. Her goal is for patients to leave FDA feeling better than when they arrived, with improved oral health being just the beginning. Dana embraces the concept of exceeding expectations, striving to make the FDA experience a category of one! She believes that marketing is fundamentally about compassion and building relationships. Dana's favorite aspect of being part of the FDA team is the collective effort to enhance each patient's overall health—body, mind, and spirit.

Dana serves as the co-chair of the Miami Regional Chamber of Commerce, participating in various committees to improve the Miami community. She is also a board member of the Boys and Girls Club of Ottawa County, where she finds fulfillment in making a positive impact on the lives of local children. She also enjoys volunteering her time whether it be with the FDA team at the Oklahoma Mission of Mercy, dipping strawberries for the Boys and Girls Club or at one of Miami’s many events.

Dana and her husband, Darian, are celebrating 23 years of marriage this summer. Together, they have six children, three of whom serve in the Armed Forces, and nine grandchildren. Dana loves date nights, practicing gratitude and journaling, and listening to a variety of podcasts. She loves traveling, kayaking, listening to music, driving with the top down, and the soothing sound of waves crashing on the shore. In their spare time Dana and her husband enjoy preparing delicious meals and spending quality time with their family and friends.
